CALA Request Form for Fund Appropriation or Reimbursement

The financial reporting year runs from July 1 to June 30. All requests for payment must be submitted by July 31 of the current year (July-June). Please allow two weeks for processing.

Note: Original receipt(s) and/or invoices(s) must be submitted to the Treasurer for any funding requests at the following address:
